Payment
Cash or checks will be accepted. Full payment is due at time of service. Insufficient funds will be charged a $40 fee in addition to the full amount of the scheduled appointment.
Insurance
It is each client’s responsibility to process forms for reimbursement from their medical insurance company. I will provide an appropriate receipt of payment for client to submit.
